Among the personnel documents, the employee's personal card is the main source of information for each employee. Purpose and types

The legislation approved a single form T2, which is maintained by all organizations, regardless of the form of ownership and the number of employees. The document contains all the basic information about a citizen:

  • information about education;
  • information about professional experience (length of service, qualifications in accordance with the education received);
  • personal information (address, family composition);
  • data on the current place of work (vacation, awards, retraining, etc.);
  • data of a passport, diploma, military ID and some other documents.

Form t2

This is the main type of document that is used in the vast majority of cases. The sheet looks like this.

Provided specifically for employees who belong to the scientific and pedagogical staff:

  • scientists with candidate or doctoral degrees, as well as with the titles of associate professor, professor or academician;
  • university professors (including senior lecturers and assistants).

The difference in form is connected with the presence of special columns (academic degree, title, date of assignment and relevant documents).




Form t2 GS

To be completed if the employee is a civil servant. The difference from the usual form is only one. Section IV appears, which contains information about the assignment of a class rank or other movements through the ranks. All other sections are filled in exactly the same way, but their numbering is shifted by 1 point (starting with the “Attestation” section, which follows under the number V).

Required documents

For correct registration, you need to take only confirmed information. You will need the following documents:

  • citizen's passport;
  • the order of the management, which approves the admission of the employee;
  • it is imperative to bring a work book (for those citizens who work for the first time, it is drawn up by the employer);
  • if a citizen belongs to the category of military servants, he also provides a military ID;
  • a document that confirms the level of education;
  • TIN and SNILS;
  • documents that confirm the passage of retraining, awarding titles, etc. subject to availability;
  • an employment contract with an employee.

All documents are provided in the form of originals.

Step by step instructions for filling

The correct execution of personnel documents is a mandatory requirement for the personnel department, therefore, when filling out, you must proceed from the official instructions, which are described in detail below.

Section I

It is the first sections of the T2 form that make up the main part, which must be filled out according to the following rules:

  1. The name of the company or individual entrepreneur is always written in full. However, abbreviated versions are also allowed if they were originally assigned to the company and reflected in the relevant constituent documents.
  2. The unique personnel number that is assigned to each employee must consist of numbers only (the maximum number of characters is 6). At the same time, the combination does not change even if the employee is promoted or transferred to work in another branch or department, etc.
  3. In the "Alphabet" column, you need to indicate only the letter with which the citizen's surname begins.
  4. Nature of work - this refers to the expected duration of the signed employment contract: more often Full time job, and if it is seasonal or a part-time job, then it is temporary.
  5. View - most often this is the main job, but sometimes a citizen works part-time (internal or external).
  6. Gender is indicated by a capital letter.
  7. Full name is written down in full, as indicated in the passport (patronymic - if available). And the date of birth is also recorded in text format, for example: "January 28, 1991."
  8. The code of the place of residence is indicated in accordance with OKATO.
  9. Citizenship is prescribed by code.

NOTE. The concept of unfinished higher education, which is also called incomplete, has been abolished since 2013, so if an employee is in currently studies at a university, only the education actually received is taken into account.

  1. Qualifications are indicated exactly according to the diploma (for example, "Bachelor of Science Education").
  2. The profession is prescribed on the basis of how it is accepted in the company - for this you need to refer to the staffing table, as well as to the order that approved the employee for this position.
  3. The total length of service is usually calculated according to the dates given in the work book. But it is also allowed to use other papers in which there is confirmed information about the terms of work of a citizen in different places.
  4. A mark on personal life is also made by codes.
  1. Under the composition of the family, you need to understand literally those people who live with a person as one family. Their full names are prescribed, as well as the degree of relationship (father, son, Native sister and so on.).
  2. Passport data must be filled out strictly as indicated in the document, up to abbreviations and punctuation marks.

Section II

In these columns, you need to use the data from the military ID, if available. At the same time, the correspondence of the data must also be strict - abbreviations, names, punctuation marks, etc. At the same time, there are some peculiarities if a reserve officer is hired.

And if an employee is hired who is subject to conscription (men fit for health and women with a military ID - nurses, some other personnel), then paragraphs 1,3,4 and 7 remain empty. The rest are filled in as shown in the table.

Other sections

The remaining sections are filled in in accordance with the following rules:

  1. ChapterIII- here all the data is the same as they are indicated in the internal documentation of the company. The full name of the company, the exact title of the position are recorded, and the order (with the name, number and date) or order is given as the basis for the decision.
  2. ChapterIV- records are also made on the basis of approved documents. In this case, each line is filled in completely (all columns), except for the column "Base". Usually here they refer to the protocol, which contains the results of certification - they prescribe the name, number and date.
  3. ChapterV- here they record the dates of training (beginning and end), type of promotion, as well as the full official name educational organization where the learning process takes place. It is not necessary to prescribe information in the "Basis", but if there is a corresponding order or order, you need to enter its name, number and date of approval.
  4. ChapterVI- in this part, information about the professional retraining that the employee underwent is similarly prescribed.
  5. IN sectionVII accounting of awards and honorary titles that the employee received for his labor merits is carried out.
  6. ChapterVIII- here information is recorded on all holidays that the employee officially received, regardless of their duration. The data must exactly match the orders, as well as the vacation schedule. Particular attention should be paid to unpaid holidays that the employee takes at his own expense. They are also taken into account regardless of the duration. In this case, all information must be the same - in the employee's statement, in the order and in the card itself (especially carefully you need to monitor the accuracy of the dates). It should also be borne in mind that days off for several hours (due to overtime, overtime, etc.) are not holidays at their own expense. And a day off for a whole working day (for 1 or more) is a vacation without pay.
  7. ChapterIX filled out in accordance with the legislation on benefits. That is, both federal benefits and those adopted at the regional level are taken into account (their list can be very different in each region). This also includes the benefits that are established by the internal acts of the enterprise (in large companies, enterprises often have their own benefits, which are established at the initiative of the employer and, accordingly, can be eliminated by his own decision). The name of those documents that give the right to benefits is indicated as the “Grounds”.
  8. ChapterX contains heterogeneous information that does not belong to any of the previously considered categories, for example:
  • information that in this moment a person is studying at a university, technical school, college, vocational school and other educational institutions; the start and expected end dates are indicated, as well as data on the actual number of courses that have already been completed at the moment;
  • data on a disabled employee: medical documents, disability group, as well as the date of change in medical decisions, if any;
  • a document-conclusion, which was drawn up by a special commission for the assessment of working conditions (harmful, dangerous factors, compliance with standards, etc.).

When filling out all sections, there is a single rule to leave the field empty if there is no data on it or the answer is negative. For example, you can’t write “I don’t have” - you just need to ignore the line.

How changes are made to the card

If any information has changed over time, they must be entered in the form in a timely manner. As a rule, the regulation provides 14 calendar days to make changes. In this case, new data is entered in the same way as in the work book. There are two options:

  1. If previously empty columns are filled in, the employee must sign, thereby assuring the accuracy of the information entered. This applies to the first and second sections.
  2. If previously recorded data has changed, then you need to carefully cross out the old entry and put in a new one. For example, if a woman is married, then the old surname is crossed out, and a new one is inscribed on top.

NOTE. In case of blots, a large number of corrections, illegibility of the text, damage to individual pages or the entire personal card of the employee, which is maintained in the T2 form, a new document is allowed, into which all information from the old form is transferred.

If an employee leaves

In this case, it becomes necessary to make entries in chapterXI. It is important here to write down the reason for dismissal, and the wording must accurately quote the articles of the Labor Code. The date of dismissal is given - i.e. the last working day of the employee, regardless of whether it fell on a weekday, holiday or weekend. The employee must sign and decipher, and if this is not possible (for example, in the event of death), then the signature is put by the responsible official or close relative indicating the degree of kinship or by the representative of the employee acting by proxy (its number and date of compilation are fixed).

NOTE. After dismissal, the card is handed over to the archive, in which it must be stored for at least 75 years.

This is where you fill in general information about the employee. To fill out, you must take from the employee such documents as a passport, birth certificate, education documents, an employment contract (or).

When filling out this section, you must not forget about indicating the codes in paragraphs 3-6. The codes are taken from the corresponding classifiers.

In clause 3 - OKATO code of that locality in which you were born.

In paragraph 4, the OKIN code for your citizenship is indicated: "1" - if you have Russian citizenship, "2" - if you have two citizenships, 3 - if not at all.

In paragraph 5, the OKIN code is indicated for the level of knowledge indicated by you foreign language: code "1" corresponds to the fact that you are able to read and translate with a dictionary, "2" - you read and can explain in this language, "3" - you are fluent.

In paragraph 6, the OKIN code for your level of education: grade 4 (primary general) - "02", grade 9 (basic general) - "03", grade 11 (secondary general) - "07", school (primary vocational) - " 10", technical school (higher professional) - "11", 3 years of university (incomplete higher) - "15", higher - "18", postgraduate - "19".

Information about education is taken from the document on education (certificate, diploma, etc.).

The section is filled out for men, information is taken from a military ID. For men who have not yet served and are subject to conscription, you need to fill out:

  • paragraph 2 - the phrase “subject to conscription” is written;
  • point 5 category of suitability - "A" - fit, "B" - fit with minor restrictions, "C" - limited fit, "D" - temporarily unfit, "D" - not fit.
  • paragraph 8 - for men who have reached the age of 50, the mark “removed from military registration by age” is put here, the phrase “removed from military registration for health reasons” can also be indicated.

The remaining paragraphs of this section for this category of citizens are not filled out. For men who are in reserve, you need to fill out all the items. All information is taken from the military ID.

The personnel officer should keep in mind that the rules for maintaining personal cards of the T-2 form, with the exception of the Instructions for the use and filling out of the primary accounting forms and documentation for recording labor and its payment, approved. Decree of the State Statistics Committee of Russia dated 05.01.2005 No. 1 does not exist, therefore the content of this article can be considered as recommendations, and not as a tangible requirement for precise execution.

Documents required for issuing a personal card; information encoding

To fill out a personal T-2 card, a personnel officer needs the following employee documents:
1) passport (or other identity document);
2) work book;
3) military ID;
4) a document on graduation from an educational institution:

6) certificate of registration with the tax authority;

All other information is entered on the basis of information provided by the employee about himself.
When encoding information in the T-2 personal card, it is necessary to follow a number of filling rules defined by the Standard for automated systems personnel management:
1. If incorrect entries or information not related to this filling are made in the coding field, the T-2 form is considered damaged and must be rewritten. Therefore, it is recommended that either exact codes, or not to enter them at all until the personal T-2 card is handed over to the archive.
2. In the absence of records or in case of negative answers ("I do not have", "I am not a member"), the encoded part of this attribute remains empty. 1 3. When specifying dates, the name of the month is written in full, the year is written in four digits; e.g. September 14, 2004

The name of the organization is indicated in full, for example, closed joint-stock company Energostroy. The abbreviated name is indicated if it is. enshrined in the founding documents. On the side, OKPO of the organization is indicated.
The employee's payroll number must include no more than six digits. It is assigned to each newly hired or employed person and does not change with any movement of an employee within the organization until his dismissal.

Abbreviations of words are allowed when writing the name of an educational institution that does not distort the meaning of the name. All information about education (qualification, direction or specialty) is established on the basis of a diploma of education.
Free lines in the education section are for the name and date of graduation of the second educational institution of higher or secondary vocational education. If the employee graduated from two secondary or two higher educational institutions in different specialties, both educational institutions are recorded. Diploma qualifications are listed in the following order:

The profession (main and additional) is indicated in full on the basis of the order (instruction) for employment. It is coded in accordance with the All-Russian Classifier of Occupations of Workers, Positions of Employees and Wage Categories (OKPDTR). An additional profession is one that does not coincide with the main profession, but may be of interest to the organization.
Work experience (general, continuous, giving the right to a seniority bonus, as well as the right to other benefits established in the organization, etc.) is calculated on the basis of entries in the work book and (or) in other documents confirming the relevant length of service.

In the composition of the family, only family members are indicated with an indication of the degree of relationship. The closest relatives are father, mother, son, daughter, brother, a sister, a person in whose upbringing the employee was, and other close relatives living together with the employee.
Passport data are indicated in strict accordance with the passport. The address of the place of residence is recorded according to the rules for recording the place of birth.
In the event that the employee did not receive a work book on the day of dismissal due to absence from the workplace or for another reason, the personnel officer sends notifications by mail to the indicated addresses about the need to appear in the personnel department for a work book (or express consent to send her by mail) or inquiries about the whereabouts of the worker. Information on employment and transfers to another job is entered on the basis of an order (instruction) on employment (forms T-1 and T-1a) and an order (instruction) on transfer to another job (form T-5). The specialist of the personnel department, when filling out this section, must acquaint the employee with the entries made against receipt.
When filling out the 4 "Certification" of a personal card, all columns are filled in, namely:
- date of certification;
- the decision of the commission, for example, "transfer to a position", "send for advanced training", etc.;
- a link to a specific document (as a rule, this is a protocol of certification results) indicating its number and date.
The "Reason" column may be blank, or it may contain an order of the organization to send an employee for certification and / or an order to approve the results of certification.
Data on advanced training are recorded on the basis of documents submitted when applying for a job or coming from the training department. In the "Professional Development" section, the following are indicated:
- start and end dates of training;
- type of advanced training;
- the name of the educational institution (institute of advanced training of the ministry (department), advanced training faculty at a higher educational institution, advanced training faculty at a secondary vocational educational institution, advanced training institute, advanced training courses at the ministry (department), advanced training courses at enterprises, scientific and research and design organizations, higher and secondary vocational education institutions, advanced training institutes and their branches);
- type of document (certificate, certificate);

Similarly, information on professional retraining is entered with an indication of the specialty (direction, profession) in which retraining takes place.

When filling out the "Incentives and awards" must indicate the types of incentives applied to the employee (both at the level of the organization and at the level of ministries and departments), as well as list state awards.

Vacation accounting

In the "Vacation" section, a record is kept of all types of vacations provided to the employee during the period of work in the organization. The basis for making entries are orders for the provision of vacations.
When filling out the vacation table on the basis of the order, only the start date of the vacation is entered, while the end date is entered "de facto" after the employee has actually returned from vacation. This is due to the fact that in the event that an employee is called from vacation or interrupted, the column "End Date" will contain the date the employee leaves for work, and not the planned end date of the vacation by order.
In the same table, leave without saving is necessarily noted wages. It is taken into account when calculating the length of service, which gives the right to an annual basic paid leave, since, according to Art. 121 of the Labor Code of the Russian Federation and it does not include the time for granting leave to an employee without pay for more than 7 calendar days.

Similarly, it is not taken into account in the length of service, which gives the right to the annual basic paid leave, and the time of leave to care for a child until he reaches the age established by law.
In addition, according to Art. 121 of the Labor Code of the Russian Federation, the time of absence of the employee from work without good reasons, including as a result of his suspension from work in cases provided for in Art. 76 of the Labor Code of the Russian Federation.

A personal card is a document that summarizes all information about a person, including an employee's biography, previous jobs, military duty and other information.

This document is the primary document of personnel records and is required to be filled out, has its own unified form T-2, which was approved by the resolution of the State Committee Russian Federation according to statistics dated 01/05/2004 No. 1.

Documents required to complete.

The responsibility for completing this document is usually assigned to personnel department organizations, in small enterprises it can be filled out by employees of the accounting department or the head personally. After the dismissal of an employee, the card is stored at the enterprise for 75 years; in the event of the liquidation of the organization, this information is transferred to the archive and continues its storage period. On the basis of what documents to fill out an employee's personal card?

This form of the document consists of 11 sections, however, filling in the upper part of the document is important. The person responsible for maintaining personnel records must correctly fill in the details of the organization and the upper part of the personal card, otherwise, in the event of a controversial or judicial precedent, the card will be invalid:

  • in this part, you need to indicate the personnel number of the employee, it is important to know that it consists of no more than 6 digits and does not change, regardless of the employee's movement up the career ladder;

How to fill out the T-2 form correctly?

To avoid mistakes, consider filling out the employee's personal card in sections.

The basic information in the first section is filled in from an identity document, then from paragraph 5 according to the employee. In cases where a person speaks three or more foreign languages, the company has the right to supplement the lines in the electronic version of the document form.

Item 6 is filled out from the submitted documents on education, regardless of the number of completed and incomplete educational institutions the personnel officer must list all the establishments.

Information about the profession and work experience is entered in paragraphs 7 and 8, respectively, based on previous entries in. The profession is entered in accordance with the staff list and the order for employment.

Also, information about his marital status and family composition in paragraphs 9, 10. According to the labor legislation of the Russian Federation, the family includes parents, husband / wife, siblings, as well as a circle of people living with the employee, the record is kept in the order of kinship.

Passport data is completely recorded in paragraph 11, the address of registration and actual residence in paragraph 12. This concludes the first section, it is important to remember that changes may occur in any data entered.

Most often, changes occur in passport data, for example, when an employee gets married or divorced.

In this case, it is necessary to carefully cross out the old surname and enter a new one next to it, then enter the basis document and endorse with the signature of the employee responsible for maintaining personal cards.

Repeat the same steps with passport data and place of registration.

Section two contains military registration information that is transferred from the employee's military ID. In the absence of a military ID, this section is skipped. If a man did not do military service and was not decommissioned by certain indicators in paragraph 2 of section 2, you must indicate "subject to conscription."

In the third section of the T-2 form, information about the employee's workplace is indicated, namely: for work, structural unit (if any), position, amount, basis for hiring (order of the head of the enterprise).

After entering the data, the employee checks the correctness of the specified data and puts his signature. When an employee is transferred to another position or an increase in salary for one reason or another is also reflected in this section.

Section 4 is filled out during the certification of employees for a change in position or transfer to another structural unit. In this section, the employee of the personnel service is obliged to indicate the period of certification, its result and a link to the certification document.

In section 5 “Advanced training”, data on additional advanced training of the employee is entered, for example, the employee had a secondary specialized education and graduated from a higher educational institution during the course of the course, which contributed to a change in his rank or qualification. The period of study and its result are also filled in. In this case, it is necessary to amend the first section, paragraph 6.

The seventh section, as well as the fourth, fifth and sixth, are filled in as needed and the achievements of the employee, as a rule, these sections are used by large organizations that send personnel to training courses, recertification or certification when transferring to another position.

In section 7 "Awards (encouragement), honorary titles» fill in information about receiving such awards as: for achieving high sales, for long service, etc.

According to the Labor Code of the Russian Federation, Chapter 19, Art. 114, each person who has worked in the organization for more than 6 months in a row has the right to take annual paid leave, leave at his own expense, and so on. Vacation data is entered in section 8 of the employee's personal card, regardless of the type of vacation.

In this section, the person responsible for filling out this document must enter the type of vacation, the basis document allowing you to go on vacation, the number of days, as well as the start and end date of the employee's vacation. When the lines in this section end, you need to report an additional loose leaf with the continuation of section eight. A new card is not required.

In the Russian Federation, there is a fairly long list of categories for providing social services. For example, a tax deduction from personal income tax in the amount of 1,400 rubles for two children and 3,000 rubles for more than a third child, inclusive. Such information is entered in section 9 of the T-2 form. In this section, you must also indicate the type of benefit provided, the number and date of issue of the document confirming the right to the benefit.

Section 10 is completed at the discretion of the employer and the existence of certain skills or medical conditions of the employee. Here you can enter the driving experience of an employee of one category or another or information about additional education not included in section 2, for example, cutting and sewing courses or receiving additional education in part-time education.

When terminating an employment contract, the employer must indicate the article Labor Code, on the basis of which the employment relationship was terminated in section 11 of this form of the employee's personal card.

Along with this, it is necessary to indicate the number and date of the order for dismissal and the date of dismissal itself.

Common mistakes made when filling out a document

Small mistakes can be corrected.

It happens that the staff of the personnel department makes mistakes when filling out the employee's personal card. This will not entail special consequences, and it will not affect the employee at all. However, if there are a fair number of errors, then during an external check, a fine will be imposed on the personnel department for poor performance.

The most common errors are:

  1. The use of special characters. [/], [:], [=] and other special characters should not be used in this document. They are especially strictly forbidden in the coding zone;
  2. Careless completion of the document. The text can overflow the boundaries of the specialized fields if needed. However, it should not end up in the coding area. If there is at least 1 letter in this area, the card is then considered as damaged, and it will need to be reprinted;
  3. dashes. This well-known oversight is due to the fact that in many other papers dashes are possible or even necessary. They should not be in the T-2 form. If there is nothing to write in the column, then it must be left completely empty.
